Advance Care Alliance of New York (ACA/NY) is a 501(c)(3) non-profit Care Coordination Organization that provides dedicated Care Managers to guide you and your loved ones as you navigate through life. ACA/NY is designated by the New York State Department of Health and the Office for People With Developmental Disabilities (OPWDD) to offer comprehensive care coordination to people with intellectual and/or developmental disabilities (I/DD).
